The Rotary Club of Toronto Transition Centre was opened in December, 1999. The Rotary Club of Toronto donated $500,000.00 to the hospital to fund this unique facility. The Transition Centre provides a temporary, safe and welcoming environment for homeless and underhoused people who have been referred through the hospital's emergency department. At the time of opening this was the first facility of this kind in Canada.

 The Rotary Club of Toronto Transition Centre at St. Michael's Hospital is located within the emergency department of the hospital and provides facilities to meet mabny of the basic needs which others take for granted each day but are rarely available to those without a home.  the centre provides a place to rest and enjoy a warm meal, facilities for showering and laundry, access to caregivers to assist with medicaltion and counselling a place to wait until transition into the community is arranged.
